Please copy the headings and paste them into the body of an email and fill them out there. 
Then send it.

(To keep prices low, we have opted for a cheaper web hosting that does not allow forms so you must do it manually.
Since it takes you about 5 seconds to open an email and copy and paste the headings,
and would otherwise add about $5 to your cost, we thought you would prefer this option.)
